Here’s a form that I adapted from everything on the site to hand my kids. I have shared it with all sorts of teachers here at school, and everyone loves it for classes where participation is key. It’s in docx form so that anyone can download and change it. I make a hundred copies at a time.

My response: This is an elegant tool in its simplicity, which is what we need. It is totally streamlined! I have seen a few of these adopted from the main rubric, but this is the simplest and therefore the fastest to process. And I am glad it gives the teacher the final say, where the box on the right would be the overide and final grade. Other versions of this as they are created would be welcome below as comments, I am sure.
